According to Schouten et al (2008), sepsis involves the activation of the coagulation cascade coupled with down-regulation of anticoagulation and fibrinolysis. An intricate link between inflammation and coagulation exists within the body (Neligan, 2006). When a pathogen is present in the bloodstream or when tissue injury occurs, an inflammatory response occurs. The response causes a stimulation of the immune system to produce interleukin-1 (IL-1), interleukin-6 (IL-6) and tumor necrosis factor alpha (TNFα) (Neligan, 2006). These cytokines are the main catalysts of the inflammatory response and cause the release of several agents including, interleukin-8, histamine, kinins, serotonin, selectins, and neutrophils.…

Medical Diagnosis: Weber-Carstens et al. (2010, p. 1) reported that diagnosis of CIM is made either clinical evidence of muscle weakness after removal of sedation as characterized by weak and flaccid extremities, in addition, electromyography (EMG) and muscle biopsy can be performed to distinguish between the different types of ICU-acquired weakness. John and Bapat (2015, p. 157) believed that EMG and muscle biopsy is impractical to perform in the ICU; therefore, he asserted that clinical diagnosis using diagnostic criteria is more feasible. Examples of these rules are as follows: Generalized weakness developing after the onset of acute illness, weakness is diffuse (involving both proximal and distal muscles), symmetrical,…

In the year 1954, a German pharmaceutical company called Chemie Grünenthal synthesized drug called Thalidomide, it was introduced as an effective sedative used to treat the symptoms of morning sickness in pregnant women. It was soon discovered that Thalidomide was in fact a potent teratogen* that could cause dysmelia, peripheral neuropathy and in some cases, premature termination of pregnancy; it was because of these reasons that thalidomide was removed from the world’s pharmacy shelfs in the early 1960’s. Today, almost 62 years after its original discovery, thalidomide is again in use, this time in an attempt to improve and potentially save the lives of patients living with acute skin conditions such as leprosy, certain types of cancer and in some cases HIV patients can benefit from this drug.…
