Once stabilized to the point that she no longer needed hospitalization, she was transferred to a SNF for rehabilitation. Following rehab, arrangements were made amongst family members and a case manager to transition Heidi into a nursing home to assist with ADLs and IADLs because she was too severely disabled to return home. Upon significant improvement, Heidi was transitioned back home and had continued care with a home health agency providing physical therapy and a homemaker. All of these services were very well coordinated and supported Heidi’s transition to self care.…

History tells a story about a time that the hospital, LTC system and mental health were all connected to one another and were all serviced in the same manner. Well, maybe there wasn’t really a hospital, nursing home or asylum so to speak of but there were people who had conditions or were poor and could not take care of themselves which resulted in a need which brought life to the health care facilities and models of care that we recognize today. Early in American history, few people lived to be old, but for those who did, “old-age security” meant having children or property. The public welfare system of those times was fashioned after the English “poor Laws”. Early on, paupers were given cash payments referred to as “outdoor relief”.…

For this assignment I am going to describe the purpose of assessment in nursing as a whole, but also relate this to my field of practice which is children and young people’s nursing. Nursing assessment can be done in many ways and also requires many different skills and approaches to be able to understand the needs of the patient in order for their health concerns to be investigated. This piece of work will look into what assessment is, how it is carried out and the reasons why it is important. Completing a comprehensive, holistic assessment enables the nurse to establish a baseline of information against of which, new and changing information can then be compared (Dougherty, Lister 2011). Being able to stay at home has been one of the many topics that my mother and father have addressed with me from an early age. Neither one of them expressed the desire to go to a nursing home or an assisted living facility. I am also so thankful that they are in good health. My father is 77 years old and continues to work in and around the home, despite some minor health issue like having both knees replaced a few years ago. He would probably stop thriving if the only option for him was to “age gracefully” in an assisted living facility or nursing home because of his age or health at the time of his knee replacements.…
