ABSTRACT
Herlyn-Werner-wunderlich syndrome is a rare mullerian duct anomaly also known as obstructed hemivagina and ipsilateral renal anomaly (OHVIRA). It is a complex of structural abnormalities of female urogenital tract. Patients are usually asymptomatic until menarche. After menarche patients present with acute abdominal pain. During embryogenesis, the Mullerian (paramesonephric) ducts represent the primordial components of the female reproductive tract and differentiate into the fallopian tubes, uterus, cervix and the upper part of the vagina. Pain was more towards the right fossa. It was insidious in onset, moderate in intensity and pain was not relieved with anti-inflammatory drugs. The patient had no history of fever, no vaginal discharge. Past and family history was not of significance. No other significant history was noted. There was no history of T.B. The patient had no history of fever, no vaginal discharge. She had attained menarche 3months ago and has a regular cycle of 28 days with flow for 3-5 days. Patient noticed a swelling in the lower abdomen towards the right side. There was no associated urinary or bowel complaints. On clinical evaluation it was found that patient was moderately built and nourished and had normal vital signs. Patient was then referred for ultrasound. USG of the Liver, Pancreas, GB, Left kidney, and Spleen were normal. Right kidney was not seen in right renal fossa, RIF and in LIF. USG of bladder was normal. Right ovary showed a cystic lesion with mobile internal echoes noted within it. Two separate uterine corpus and fundus was noted. Right uterus showed distended endometrial cavity filled with fluid containing mobile internal echoes. Right adnexa showed ill-defined saccular elongated hypoechoic structure with septation and mobile internal echoes within
