H. History of Need or Problem: Child has a history of violence – first criminal charge occurred at 14. Attempted to commit suicide at 14 and was hospitalized. November 2015, adolescent entered juvenile detention for assaulting her brothers. Child has a history of fighting her siblings, classmates, and teachers. Child removed from home when she pressed charges for assault against her mother’s boyfriend.

1. Strengths: Client has access to services and resources. Adolescent is outgoing, receptive, aware of situation, and maturing.
