Gordon's Functional Health Pattern Assessment

The Gordon’s functional health pattern assessment consists of 11 categories that nurses can use to identify risk factors that can lead to potential dysfunction (Edelman, Kudzma, & Mandle, 2014). The nurse can use this information to alter elements to influence a healthy outcome (Edelman, Kudzma, & Mandle, 2014). Additionally, this assessment allows families to analyze aspects of the family that they would not normally consider. An interview with Smith family using Gordon’s functional health patterns model will be for reviewed for any risk factors related to health and used to identify 2 nursing diagnoses that a nurse would use when caring for this family. When questioned about the health-related behaviors that the family participates in, they mentioned refraining from alcohol and staying physically active by doing work around the house. The wife has a job at a grocery store and attributes that to her physical activity as well. The family reported to not have any harmful behaviors or habits that impact health but have a family history of smoking, drinking, and drug abuse which has resulted in mortality.
Nutrition
The Smith’s beliefs on diet are everything in moderation and do not follow anything in particular. Trans Fat and genetically modified foods are the only concern when shopping for groceries. The couple eats breakfast and dinner together and the same time every day.
Sleep and Rest The Smith’s follow the same sleeping routine daily which the wife sleeps from 11:00 PM to 5:00 AM and the husband sleeps from 10:00 PM to 2:00 AM and frequently naps during the day. They both reported waking often during the night due to muscle cramps, dogs, cat, alpaca, and coyote. The wife drinks a cup of tea each evening before bed to relax.

The assigned diagnosis is disturbed sleep pattern related to noise and caregiving responsibilities as evidenced by reports of being awakened and frequent daytime napping. He only sleeps 4 hours in the evening and frequently naps during the day. The lack of sleep may be the cause of his sedentary lifestyle or the sedentary lifestyle is the cause of his disturbed sleep pattern. The issues seem to be a cycle which leads to the next wellness issue to be discussed. The second wellness problem identified is the husband’s sedentary lifestyle. The assigned diagnosis for this issue is Sedentary lifestyle related to lack of interest as evidenced by choosing daily routine lacking physical exercise. The husband spends many hours watching television or using the computer which could be leading to his sleep disturbance. His lack of physical activity can lead to many other health
