1-2% of lidocaine or 0.25 or 0.5% Bupivacaine 10ml of either medications is used. Lidocaine needs at least 10 mins to be effective. Used a 10 ml syringe with 27 gauze with ¼ in length needle. Lidocaine with epinephrine can use to decrease bleeding and extent the Lidocaine effect. However, epinephrine is contraindicated with single circulation or end such as fingers, toes, ears, and nose. After cleansing the skin with an antiseptic a local anesthetic is applied. The needle is inserted with bleb up under the dermis of the entire wound. Because it is a subcutaneous procedure watch for a flash of blood, and then slowly infiltrate wound as you pull back out with the needle. This is done on each side of the wound. Need to wear gloves, gown, and a face mask with shield. Used sterile technique when possible, although it is not necessary, to prevent further infection.
Procedure: use 11 or 15 scalpel blade, hold the blade between the thumb and forefinger to make the initial entry directly into the abscess. The incision should be directly over the center of the cutaneous abscess, and along the long axis of the fluid collection with steady firm pressure. The flow of purulent drainage will begin to flow if the abscess cavity is entered successfully. Control the scalpel as you make the incision to prevent puncturing through the back of the wall to prevent uncontrollable bleeding. The incision opening should be large enough and extent to the borders of the abscess to ensure adequate drainage, prevent recurrent of the abscess, and to promote healing. After the wound is drained spontaneously, gently used the pads of your fingers to express any further
