Reason for visit: FB to right thumb
S: Hanwha TM is here with FB in his right thumb. According to the TM he was working under cover, when he received his FB abject to his left thumb. TM denies any other issues. TM doesn’t recall the last tetanus shot. TM agrees to all procedures and signed the consent.
O: Right thumb with FB, small, pin prick size, no edema, no bleeding. Removed the FB form the right thumb, hair thin, 2 mm size FB, no bleeding, with minimal pain, TM tolerated well

Clean the hand with Hipiclens, pat dry, clean the right thumb with alcohol pad, pull the FB out with tweezer, antibiotic ointment and Bandai applied Instructed to follow up with Medical Clinic as needed, s/s of infection reviewed with the
