Clinical Context & Experience I completed my final two shifts on 11/24 and 12/1 at Sharp. The unit had a pretty large census both nights, so we were fairly busy both nights. During this shift I had to notify the doctor twice and felt the most relevant journal topic was advocacy. The first night I had three patients which included a 84 year old man who sustained a hip and right shoulder/humerous fracture from a fall at home and a history of HTN, CKD, MI with coronary stent placement, and diabetes, an 86 year old woman admitted for rapid A-fib with history of TIA, HTN, CHF, and rheumatoid arthritis, and finally a 35 year old woman who was being discharged following her treatment for polynephritis. Effective transfer of patient information from nurse-to-nurse is vital to ensure safety and positive patient outcomes. Wakefield, Ragan, Brandt, and Tregnago (2012) emphasized that bedside reporting has been linked to better patient involvement, improved effectiveness of communication among nurses, and enhanced nursing teamwork and accountability. By relocating the shift handoffs from the nurse’s station to the patient’s bedside, the patients can be more involved with their care, the nurses are better able to prioritize their tasks, and the nurses will no longer have to rely on the report given to them because both outgoing and incoming nurses can compare their assessments. The AACN suggests nurses nurse staffing must ensure an effective match between nurse skills and patient needs (AACN, 2016). Not only does inappropriate staffing lead to medical errors, it can also bring about stressed and overworked nurses. The unit I work on is often short-staffed leading nurses to take on more than they are used to and leaving charge nurses scrambling to assign duties and calling in reinforcements. To the best of my ability, I will try to match my nurse’s abilities to the patient’s acuities and request critique if the assignments do not work effectively with current staffing.
