1. Who on your senior team should lead this effort?
The effort to change any policy in the hospital will start with the CEO and the Board of Directors. When they have analyzed the situation, they will develop a plan in order to change the way the service is provided. Then the plans will be given to all the senior management in order to instruct the employees on the new system.

What can they do in order to add to the patients benefit, decrease unnecessary payments or losses in finances, decrease patient harm, and lower the amount of unnecessary readmissions, such as in the case of chronic illness.
4. Who will help communicate this effort to all frontline staff?
The manager of your department will communicate this new strategy to the employees, on the floors it will be the manager of the floor, then each charge nurse of every shift will go over this again. Most questions can be handled by a manager or charge nurse, if not then they will submit the question to senior management.
5. What kinds of systems need to be created to track progress?
The hospital must track the new policy and system, they are unable to make any recommendations if they do not have proper data, numbers, and measurements. Every department will measure how they are doing in helping with the effort. Every floor will have measurements and goals that they will be striving to achieve. Every floor will track how many patient falls they have for every shift, they will track IV sites and check for infection. Pressure ulcer protocols will be put in place for all susceptible patients, and a protocol will be given for patients who are at an increased risk for blood clots. The patient needs to have clear instructions and education about their condition. The patient should be able to receive their medication at the hospital pharmacy, and given the proper directions on when to take them. The patient should have a social worker evaluate their home situation, to see if they will require additional help, such as a visiting nurse or a physical therapist. The primary care physician will need notification from the hospital and he will need to work with the patient in order to make sure they are working towards recovery. Hospitals will also have a nurse call the patient once they are home and check to see how they are progressing, and we may incorporate technology where the patient may be monitored at home.
