Four of the nine errors (44%) originated from the branches due to human error. The remaining five of nine errors (56%) originated from partners. Additionally, Document Management did not detect the error due to human error. Regarding Document Management the human error was impacted by the demising effectiveness of the review as employees reviewed 1,200 transaction during a work day.
