Describe The Role Of A Case Manager

I am a case manager in an insurance company. There are three people, nurse, social worker and assistant to form a team in each group in my company.
The majority of my job is to provide support or assistance to my members. Also, we require reviewing nursing visit note, progress note as well as discharge note from hospital or nursing facility. In addition, we need to issue a lot of tasks, such as durable medical supplies, medical equipment and so on. I only delegate tasks with low risk, such as durable medical supplies, medical equipment, and so on to my assistant after reviewing the request by health care provider or members. Also, I delegate the task of arranging a regular car service to my assistant; not Ambulance or Ambalette service.
