At that same time, Harold’s sister was in a car accident, left bedridden, and permanently in need of care. Six months later he and his wife decided to spice up their relationship and invited a man to participate in their sexual life. She confessed two weeks ago she had feelings for this man and had been seeing him without telling Harold. Harold became angry, started drinking again, stopped self-care, became withdrawn and sad, discontinued leisure activities, and began having suicidal ideation. The groups offer open enrollment. Since the insurance company agreed to thirty days of treatment for Harold, he is placed in the higher functioning therapy group consisting of members with diagnosis of depression, anxiety, bi-polar, complicated grief and those with suicidal ideation or attempts. He will participate in the group five days a week for the first two weeks and begin decreasing weekly sessions to three days a week with progression. Harold is set up to visit with a nurse and psychiatrist weekly. The psychiatrist prescribed Effexor for his depressive symptoms. Harold’s treatment goals are to decrease depressive symptoms, identify stressors in relationships, decrease thoughts of suicide, and refrain from alcohol use. Greenbrier staff will monitor Harold for expressed suicidal thoughts and continue to evaluate the treatment plan and
