On December 4, 2017 at 4:00 pm, I Cora Williams arrived at Clearr Vission. Upon entering the building, I observed Mr. Logan walking into the conference room. I informed Mr. Logan that I had just returned from Buffalo, NY due to a funeral therefore I was a little tired. Mr. Logan inquired if I wanted to reschedule. I advised, “no.” As I proceeded with the documentation training, Mr. Logan grew upset. I began explaining to Mr. Logan that his notes were not consistent. Next, I reviewed several of Mr. Logan’s notes side by side to show the difference in his writing style. Then, I provided Mr. Logan with examples to use within his writing. As I explained to Mr. Logan that quality in addition to the quantity of information he places in his notes will justify the hours

Mr. Logan explained that he uses the correct amount of interventions in his notes. Mr. Logan also expressed that he did not see anything wrong with his notes.
I reviewed Mr. Logan’s notes again showing specifically what changes needed to be made. Mr. Logan verbalized again that he did not see anything wrong with his notes. I explained to Mr. Logan that if the changes were not made according to how he was being trained, all his notes would be returned. Mr. Logan replied, “Oh well, I’ll just send them back.” AS I made another attempt to show Mr. Logan a simpler way of correcting his notes he became rude commanding, demanding that I “come on and hurry up.” I advised Mr. Logan to reframe from speaking to me in a condescending tone. Mr. Logan’s voice grew louder as his speech began to increase. At this time, I informed Mr. Logan that training was over, if he refused to be quite and listen. Mr. Logan continued
