Conxus Profile: Health Risk Assessment

ConXus Profile is a health risk assessment for adults (18 years and older), including commercial populations, seniors (Medicare), and low-income groups (Medicaid). It is Wellness and Health Promotion Health Appraisal Certified (WHP 5) from the National Committee for Quality Assurance (NCQA).

ConXus Profile supports question completion online from any PC, tablet, or smartphone. The questionnaire may also be completed using paper forms or facilitated by a health navigator. Laboratory results and measurements can be collected at health fairs, central labs, or using provider forms and loaded before or after the questionnaire is started. Biometric results and are consolidated with the questionnaire responses to provide a comprehensive assessment.
