This experience occurred during either the first or second week of clinical wherein I had experienced a patient turning hyperglycemic. The patient in question was a 78 year old gentleman, diabetic type II, who was admitted into the system due to a fall causing a left hip fracture. He then was subsequently transferred after surgery for rehab to regain his balance and strength; arrived to the unit.

The event occurred 10 days after his arrival on the unit, when I was going through the morning routine check-in: before helping him get ready for the day. I walked into the room ready to check his vitals and see how his rest was the night prior. He was

I had quickly gone through a pain assessment with them and then searched for the clinical instructor for their judgement in plan of action. Just as similarly, I was flustered at the deteriorating health status of my assigned client as I use best judgement that my abilities at the time allows me to intervene; from promoting health to providing distraction for pain a la warm tea and talk.

Taking this deeper into analysis, in both cases an adverse reaction occurred in my assigned clients and this change in status under my care took me by surprise. I was not aware of the possibility of these changes in health to occur so suddenly, nor the signs and symptoms being asymptomatic. This is a clear indication of an insufficient ability to fulfill the prevention aspect of nursing by means of creating connections and pattern recognition to foresee possible changes in health
