One of the main communications formally that the hospital has into place is called, “rounds.” Rounds are when the Hospitalist and Regristered Nurses, and Nurse Case Managers and the Director of Clinical Resource Management gather in a room every weekday at nine o’clock AM to discuss patient status and their discharge plans. The informal communication that they have with one another, I believe is just having eachother personal contact numbers, therefore if one of them have a question or an issue they can be on-call. Where I think could be beneficial for the patients, is that the Hospitalist and other professionals communicate more often about the more difficult patients, and that they come to an agreement about the plan of action for patient so that they won’t confuse the
