DOI: 7/15/2011. The patient is a 55 -year-old female cashier who sustained a work-related injury to her low back from performing repetitive duties. The patient is subsequently diagnosed with degenerative disc disease, lumbar spine and myofascial pain. As per office notes dated 6/14/16, the patient present low back pain. The patients states that she is feeling worse. She is having increasing pain in her low back and left lower extremity. She is complaining of increased pain in her hands. Pan level is at an 8/10 level. She is trying to do exercises. She would like trigger point injections. During physical examination of the thoracolumbar spine, it was noted that the patient is depressed and anxious. Palpation revealed tender trigger points over
