Describe the Quality of care being provided- During the visit Franklin was dressed in a t-shirt, sweatpants, baseball cap and flip-flops. Franklin appeared neat and was in a pleasant mood. Franklin’s apartment shared with a roommate appeared very neat. Roommate was asleep during the visit.
Health- Franklin was recently involved in a car accident and as a result of that his arm was in a sling. Franklin went to the hospital and was given prescriptions, which he did not take. Franklin explained that he removed the sling and was no longer in pain.
Service needs- Franklin currently does not have a bank account. Opening a bank account was discussed with Franklin and he was open to looking into a bank account with M&T Bank, which is close to where he resides. Franklin is currently applying for jobs and so far has applied to be a Patient Transporter at Northwest Hospital in Baltimore County. Franklin has also applied for Target, Amazon and Walmart. Franklin agreed that he will follow-up on the positions he applied for.

Ms. Butler was meeting with Franklin to come to an agreement that Franklin is ready to establish paternity for his 4 month old daughter Lyric. Now that Franklin is ready to establish paternity he will go to vital records with the child’s mother and update the birth certificate. However, before the birth certificate can be signed Franklin and his child’s mother will have to sign an affidavit. The affidavit will be provided by Ms. Butler the affidavit per Ms. Butler will be provided by October 16, 2015.
