Wellness Coach Teresa Cain met with Mark Kopolovicz to conduct an apartment inspection discuss Medical Health as well as mental health.
Mark Kopolovicz scheduled apartment inspection was on 12/13/2016. Wellness Coach went to Mr, Kopolovicz residences and he open the door appearing to be alert but refused to let Wellness Coach complete an apartment inspection. Wellness Coach stated that if meeting in the staff office would be a preferable option and Mr. Kopolovicz stated he would come down to the staff office.
Mr. Kopolovicz did not come down to the staff office that day but he came the staff office 12/15/2016. Wellness Coach asked Mr. Kopolovicz did his apartment need any repairs and his response was no. Mr. Kopolovicz is known to have hoarding

Mr. Kopolovicz was referred by HUD Vash to receive an APS case worker to assist in his overall ability of enhancing his life. The concerns was about his hoarding, diabetes and rental arrears. Ms. Alfaro along with Wellness Coach knocked on Mr. Kopolovicz residence but he was not home therefore Ms. Alfaro left a note for an upcoming appointment 12/29/2016 at 10:00am. Mr. Kopolovicz came to the staff office later that day and informed Wellness Coach that he received the appointment and will meet with Ms. Alfaro. Wellness Coach said okay that is good. On 12/30/2016 Mr. Kopolovicz came to the staff office alert and groomed. He informed me that he wish not received services from JASA and he told wellness coach that he do not bother no one, he do not do drugs or drink and pay his rent. Wellness Coach explained that the APS worker was to provide supportive services to assist in his daily living of life. They are put in place to make sure your bills are paid and if you need assistance in cleaning or money management they are there to help. Mr. Kopolovicz said he do not need those services and asked to use the staff phone. On the phone call he canceled another NYCHA apartment inspection. Wellness Coach offered assistance in organizing his belongs but he continuously decline. Wellness Coach will follow-up with APD Kia to find other ways in which we can assist Mr. Kopolovicz in his habit of
