Case Report: The Field Investigator

On Monday, November 21, 2016, at 5:00 a.m., the Field Investigator traveled to meet with Witness Mr. Jeffory Christ Johnson at Frontier Communications, at 3770 Industry Avenue, Lakewood, Ca. to receive his recorded statement about his observations of the claim involving the DOL May 9, 2016.

Mr. Johnson provided his version of the accident involving his company vehicle which he was assigned when he heard a loud “pop” as he drove forward past the claimant’s vehicle as it made a right-hand turn on to Anaheim Street from Pacific Avenue, in the City of Long Beach, CA.

Mr. Johnson reportedly pulled over to the curb across the street from where the Point of Impact occurred and contacted the Claimant who complained about having lower back pain
