On 5/24/2016, CM met with the client and completed Bi-Weekly ILP Review. In the meeting client was dressed appropriately for the weather. Client appears to be cooperative, friendly and forthcoming with all information.

SOCIAL SUPPORT UPDATE: Client continues to report she need to surgery in the stomach to remove hernia and residing in the shelter, she is unable to go through with the surgery because she will need a HHA help.

EMPLOYMENT UPDATE: Client continues to report she is WECARE/FEDCAP exempt.

RESOURCES UPDATE: .Client continues to report she is receiving SSI $$733.00. Client is aware she must submit an update SSI Award letter.

MEDICAL UPDATE: Client continues to report arthritis in her left leg and hand, high blood pressure, stomach hernia and hip replacement surgery. Client continues to report she needs surgery and she can wait to be housed so she can schedule both surgeries stomach and hip replacement.

She mentioned at the present she is only taking: Gabapentin 100mg. Client reported next upcoming orthopedic or pain management appointment is scheduled for 5/25/2016.

MENTAL HEALTH UPDATE: Client continues to report she is receiving mental health counselling at After Hours Project. Ms. Christina Coon/Counselor tel# 718-249-075. Every Friday @ 2pm. Client reported she is learning coping skills.
SUBSTANCE ABUSE UPDATE: Client reported she had a few beers and she is denial of her substance
