Well, whether your office is ready or not, ICD-10-CM is coming. While its implementation has been delayed over the past few years, it's not going away. This update is more complicated than the ICD-9-CM. Things change. Diagnosis and treatment both have gotten more complicated in the past 25 years since the ICD-9 was introduced.…
Marilynn E. Doenges, Mary Frances Moorhouse, and Alice C. Murr describe the further process in the Nurses' Pocket Guide. Renal failure is extremely dangerous for the fact that when the kidneys shut down, dangerous wastes can build up in the blood that is now not being filtered.…
Organization Name & Description The organization I have chosen to develop a communications plan for is the Ontario Renal Network (ORN). The ORN, a division of CCO, and an agency of the provincial government, manages the delivery of chronic kidney disease (CKD) services across Ontario (Ontario Renal Network [ORN], n.d.a). The ORN structure consists of a headquarters located in Toronto, Ontario, that is co-led by a non-clinical Vice-President and a nephrologist Provincial Medical Director.…
Medical Diagnosis: Weber-Carstens et al. (2010, p. 1) reported that diagnosis of CIM is made either clinical evidence of muscle weakness after removal of sedation as characterized by weak and flaccid extremities, in addition, electromyography (EMG) and muscle biopsy can be performed to distinguish between the different types of ICU-acquired weakness. John and Bapat (2015, p. 157) believed that EMG and muscle biopsy is impractical to perform in the ICU; therefore, he asserted that clinical diagnosis using diagnostic criteria is more feasible. Examples of these rules are as follows: Generalized weakness developing after the onset of acute illness, weakness is diffuse (involving both proximal and distal muscles), symmetrical,…
Kidneys for Sale Miriam Schulman is the assistant director of Markkula Center for Applied Ethics (hereinafter referred to as “Center”). In 1988, the Center posted the article titled “Kidneys for Sale’’ on its website, highlighting the ethics and morality issues surrounding the continued sale of human kidneys for personal profit, which also led to emails from destitute people who wanted specifics on how they could sell their kidneys, which compelled The Center’s Issues group to discuss the pro- ideology and the ethical issues concerning the issues of organ sales. Overall, the article discusses three main considerations about the sales of organs: the morality and ethics of such exchanges; the true shortage of human organs available…
As of October 1st 2015 the International Classification of Diseases 10th Edition (ICD-10) is the national coding standard in the United States, as mandated by the Department of Health and Human Services in 2009. The International Classification of Diseases is an alpha-numeric coding system to describe patient’s conditions, symptoms and diagnosis for payers, providers and researchers; essentially providing a universal language for medical necessity and statistical analysis. The World Health Organization creates the system and governments around the world adopt it. The previous addition was adopted in 1979. ICD-9 no longer offered enough capacity in categories and codes to encompass variations in medical practice and to reflect the differences…
DOI: 5/28/2012. Patient is a 64-year-old female home attendant who sustained injury when a client put her motorized wheelchair in reverse and it ran over her right foot, pushing her up against the wall. Per OMNI, she underwent a right knee arthroscopy with partial medial and lateral meniscectomy, chondroplasty, synovectomy and joint debridement on 03/18/13. Based on the progress report dated 03/03/16, the patient is currently on PT 1-2 times per week.…
The setting for this intervention is a clinical setting where the victim will have a medical assessment. Psychical exam will be done and a sexual abuse medical evaluation will also be administered for the laboratory tests. Also, the victim will be doing assessments which include obtaining background history and determining the needs of the victim. This may include assessing the physical, emotional, and behavioral consequences of sexual abuse. By doing the assessment, it will help correlate the care for the child and show a better understanding of the child.…
Two different CT scan with contrast media performed can result to contrast-induced nephropathy due to preexisting decline in renal function. CIN can be defined as sudden, rapid deterioration of kidney function due to parenteral contrast administration. Contrast-induced kidney injury is diagnosed by an increase in serum creatinine of 25% or more within 48 to 72 hours following administration of contrast. Mr. W’s creatinine level increased from 0.8 to…
Practice Dimension Criteria: Practice: Ms. Upham provided strong leadership in the planning, coordinating and implementation of the Peritoneal Dialysis Program. This is a brand new program, developed to bring back the Veterans who were receiving their care out in the community. With the complexity of their care due to multiple co-morbidities it is critical for them to receive all their care at the Providence Veterans Administration Medical Center (PVAMC). Evidence based resources were used in creating the program and the Peritoneal Dialysis International standards were used. Objectives were written and goals put in place.…
Common leg injuries are some of the most inhibiting and destructive injuries that can occur. Countless studies have been conducted to reveal if there is a way to possibly prevent these occurrences. Symptoms often develop from everyday usage, overuse, or an injury. Leg injuries are most likely to occur from sports activities. Most of these leg injuries in children and teens occur from playing sports or falls.…
Sepsis continues to be a significant cause of death and morbidity in the intensive care unit (ICU), with documented mortality rates as high as 50% (Nargis, Ibrahim, & Ahamed, 2014). Moreover, it is also estimated to be the principal cause of death and illness worldwide (Singer et al., 2016). The socioeconomic costs related to sepsis in the United States were determined to be more than $20 billion in 2011 (Balk et al., 2017). Also, there has been a steady increase in the incidences of sepsis as the technological advances in medicine allow for more invasive procedures and the growth of multidrug-resistant organisms become more prevalent (Nargis, Ibrahim, & Ahamed, 2014).…
Critical care is a component of acute care, and it aims to treat patients who require high or constant levels of monitoring and support. These patients are often in life threatening positions, and are cared for in Critical Care Units (CCUs). Patients who enter critical care can be split into two groups, planned and unplanned admissions. A planned admission may be a patient undergoing an intricate elective surgery who requires one to one monitoring peri-operatively. Unplanned admissions can be further divided into patients entering hospital in a critical condition (an example would be severe trauma) and those whose condition deteriorates into a critical state after admission (an example would be developing severe sepsis after an operation).…
Acute renal failure is the sudden dysfunction or disease of the kidneys that can significantly impair renal function within the body. According to Huether and McCance (2012), renal failure is characterized by enough of a decline of renal function to require dialysis. Renal failure is after renal risk and injury, but prior to renal loss and end-stage kidney disease (Huether & McCance, 2012, p755). The dysfunctions the lead to acute kidney failure can be caused due to sudden renal injuries or by systemic diseases of the body. The major issue with acute renal failure is the fact that the kidneys filter the blood so it is linked to every other part of the body and can spread diseases more readily which could make it easily life threatening…
Kidney Function Introduction and definition of terms: The kidneys are the main organs in the urinary system. They filter waste products out of blood from the renal artery. These are then excreted. Useful solutes are reabsorbed into the blood. They also have a major homeostatic role in the body, and help to control the water content (osmoregulation) and pH of the blood.…
