The client is a 30-year-old Brazilian, married, homosexual male, has no children was referred by his husband to the Evolutions Treatment Center for his excessive drug usage.

Mr. Sigesmundo was born in Aracruz Esprito Santo, Brazil and raised by his biological parents. The client reported his childhood was "difficult," and "poor," because he did not have much growing up. He noted his father was not present in his life during his early upbringing until the age of 13yoa, because his work required him to travel. He described his father as being short tempered and his mother as caring. He noted that he and his sisters communicated while growing up, but two of his siblings refused to talk to him because of his sexual orientation. However, he

He stated he has anxiety, but denied he was diagnosed with an Anxiety Disorder. He noted he is taking Trazodone for his anxiety. According to the client, his mother was diagnosed with Depression. The client reported he was baker acted two weeks ago prior to this intake. He shared he had a fight with his husband, was high and his husband was afraid he would harm himself. He denied having any suicidal ideation, thoughts, plans/intent during this time. According to him, he was discharged the same night. He denied another other family members having a mental health problem. The client has experienced a significant amount of trauma in his life from an early age. He reported he was physically abused by his father while growing up. Additionally, he reported he was raped on two different occasions, at age 6 and 12-years-old by a younger and an older male child, but denied these individuals were punished for their actions. He stated when his father found out about the incident, he was beaten with the iron's electrical cord and it was never spoken of again. Again, the client experienced being bullied in school, at church and in the neighborhood because he was different and others would tease him because he was different. He reported his father would beat him for being bullied. He further noted he was sexually molested between the ages of 11-15 by older men for money to play video games. He would drink three mixed drinks on weekends, his last was July of 2017. He noted his first occurrence with drugs was 19-years old, he experimented with Ketamine, Ecstasy then GHB, which he was unaware what exactly it was. He stated he was at a party and was giving GHB, which he accidentally overdose on GHB and red bull and ended up in the hospital overnight. The clients addiction with drugs continued into his adulthood. He has tried Adderall, LSD, Marijuana, GHB and among others. His current DOC is Crystal Meth, which he inhales, smokes and has rectal administration 3/4 of a gram everyday. He reported he has tried to stop in the past, the longest was two to three months, but would eventually relapse. He noted he is mostly triggered when he and his partner have verbal arguments. He acknowledge his addiction has caused him to stop exercising regularly and emotionally compromised his value systems.The client noted this is his first formal substance abuse treatment. He denied ever participating in any AA/NA or 12 step
