City brings her 6-year-old child to see her primary care physician, complaining that her child has seemed “off” for the past month. The child has been extremely fatigued and taking multiple naps during the day that do not seem to refresh her. She is normally a good sleeper at night and recently has begun to have trouble with multiple awakenings. The child has also complained multiple times of frequent headaches throughout the week. She has also been seen to exhibit some confusion while playing in the woods in their backyard with her friends. The mother has noticed the child has had fevers and chills on and off for the past month. The child had no complications at birth and does not have a significant past medical history. With the child being from a Northeast U.S. region such as Connecticut and playing frequently in the woods in her backyard, Lyme disease is a highly likely cause of her symptoms. Some common symptoms in pediatric Lyme disease cases include severe fatigue unrelieved by rest, insomnia, headaches, nausea, abdominal pain, impaired concentration, poor short-term memory, inability to sustain attention, confusion, uncharacteristic behavior, outbursts and mood swings, fevers/chills, joint pain, dizziness, and noise and light sensitivity. Fewer than 10% of patients may present with an erythema migrans rash (bull’s-eye rash). First-line treatment is oral doxycycline, amoxicillin, or cefuroxime, which have equivalent efficacy for the treatment of Lyme disease. Doxycycline is not recommended for children under the age of 8 because tooth staining may occur. It
