This care approach is based on patient centered, continuous, and comprehensive care. Medical homes focus on coordination and integration of care, quality and safety, enhanced access to care, and an open schedule with extended hours. It promotes open communication between patients and care providers. The patient centered medical home care model has improved the quality of care, patient satisfaction and positive experiences while reducing hospital admission and emergency department visits (Bleser, Miller-Day, Naughton, Bricker, Cronholm, & Gabbay, 2014). The focus of this care model is the prevention and management of chronic disease, promotion of health and wellness in the community by collaborating with multidisciplinary team members, community members, and social services. The nurse managed health clinics are managed by advanced practice nurses to provide efficient and cost effective primary health care and/or wellness services, including prevention, early detection, and management of chronic disease, along with the promotion of health, teaching and counseling to the underserved population (American Association of Colleges of Nursing, 2010). The managed care clinics provide services regardless of income or insurances with the association of the university?s department of nursing, government?s qualified health centers, social services agencies, and nonprofit health care organizations. These clinics are well trusted by community members because of their comprehensive and community based, patient and family centered, holistic and culturally competent patient and family care (Burke, & Schwartz, 2012).
